Auction Item Donation 

Thank you for donating an item or package to the Lifesavers Gala. Please complete the below form. We will be in touch with you if we need additional information.

Please Note: We reserve the right to combine your item with another package. All donated items must be new. Exceptions will be made for genuine antiques or collectible items. New items must be in original packaging and collectibles must come with a certificate of authenticity. Gift certificates must cover the full cost of the item. We do not accept certificates that represent a percentage off the full price of an item or a certain dollar amount toward the full price of an item.
